Hour Reporting Forms

The Work Study Program allows students to earn credit towards graduation for work experience related to an academic course of study.  This program is available to any high school junior or senior student who is actively employed.

These students can earn up to two credits towards their graduation.  These credits can be earned from a placement; before school, after school, or up to three hours during school.

The information presented here and the associated links are to assist you in having a successful placement.  Please know that my door is always open to you for answers to questions or to help you with problems that my arise.  Feel free to contact me at any time!  Email seems to be the best way to make that connection and often provides the quickest response from me.

Now that you are enrolled in the Work Study Program, you have several responsibilities that you must complete within the first 48 hours of your placement.  Once you have completed these several responsibilities, you will have limited things to complete for me.  The majority of you will be shifted to your employer.  However, there are some policies that you should be aware of and these can be viewed by clicking on the Program Policies button on the left.

When you received your Training Agreement you also received an information sheet that outlined a few tasks that required your immediate attention.  Outlined below are those tasks.  Please click on the links, review the information and in some cases complete the included forms.

Immediate task list:
1.  The Training Agreement
2.  Secure required signatures
3.  Safety training seminar or video
4.  Rules and Responsibility Form

There is one additional task that you must complete for me to fulfill your requirements of this program.  That is the completion of weekly hour reports.  This one item is the only one that I grade, all other grading is completed by your employer.  However, this is a big one.  Should you fail to complete these hour reports, you will fail the Work Study placement.  The Hour Reports may be accessed by clicking on the Hour Reporting button in the left margin.  I will also remind you of these reports and deadlines via email.

The Training Agreement

The Training Agreement is a contract between you, your employer, and myself.  By placing our signatures on that Training Agreement, we all agree to the responsibilities we have.  Yours are outlined in the Rules & Responsibility section.

Please review this Training Agreement and make certain that all the information provided is accurate and true.  If there are inaccuracies, please bring it to my attention and they will be corrected.

Secure the required signatures

This contract is invalid if the required signatures are no affixed, therefore you are required to collect four (4) signatures.  These four signatures include: yours (Trainee), your parent or guardian's (Parent), your guidance counselor's signature (Counselor), and your Employer's signature (Employer/Supervisor).  This task will require you to do some running!  Please complete this task within forty-eight (48) hours!

I will take care of all additional signatures!

Safety training seminar or video

As a part of your Work Study placement, you are required to receive introductory safety training.  This requirement will be completed in one of two ways.  One of these may be your participation in a safety training seminar.  In this case, you will receive notification of the training seminar and you will be required to be in attendance.  This training will take place during school hours and you will be released from and classes that may interfere with the training.  This release will not count against your attendance.

The other way the training may be provided will be through the use of a video presentation on video tape or DVD.  If you are given this option, you are required to view the presentation that evening and return the tape/DVD the next school day.  To ensure the return of these tapes or DVDs, you may be assessed a $15.00 replacement fee for any lost, missing or late tapes/DVDs.  Please be prompt in returning these!

In either case, please initial and date the Training Agreement that you have received the safety training.
